Thank you for your interest in the Minnesota Work-Life Champions™ Awards. Please note that the application process is now closed. How to Apply User-Friendly Process Special Note: The online application process has been improved to make it more user-friendly. Applicants will be able to save and re-enter partially completed applications. Note for Small Employers (5-49 employees) you may use a simplified essay application. Please note, that once you have started entering information on this particular form, you cannot save and re-enter at a later time. We suggest that you set up the essay (or essays, if you wish to submit a short essay on an Innovative Practice) in a separate document, review it and then paste it into the essay text box below in this online form. To Apply To apply for the Champions Awards, please first read the section on About the Awards and then click on one of the links in the left column on this page to access the actual application. The Application As you begin an “online application,” the system will prompt you for basic information and a password that will enable you to enter information, save what you’ve entered and return on later occasions to enter additional information. Note: If you save before a page is complete, you will lose the information you have entered on that page. You must complete the page you are working in order to save the information you have entered. Carefully review and answer each question in the survey. Information provided should relate to practices, policies and programs that pertain to your Minnesota-based employees and location(s). If you have several Minnesota sites, please combine them for this application. Please print a copy of your final application for your own records prior to submitting it for review. A PDF of the application form is available by clicking on the "PDF Version of the Application" in the left column of this page. After opening the PDF, you may enter information before printing it. When entering text, please observe the word count limits. Text exceeding those limits will not be accepted by the text box. Special formatting (tables, etc.) may also cause text to exceed the space limits for text boxes. Alternatively, simply print out the PDF and complete it by hand and attach text where needed. PDF applications must be printed and mailed to the Center for Ethical Business Cultures (CEBC), 1000 LaSalle Avenue, TMH 331, Minneapolis, MN 55403-2005. CEBC will provide you with a confirmation of receipt of your application. Please note that the deadline for submitting an application has been extended to November 8, 2007. Employee Survey After submitting an application, employers will be contacted by the Awards program and provided with a short employee survey to be sent to a small number of randomly selected employees. The survey can be completed online by employees and is submitted directly to the Awards program.
